Information You Need for MDAC
The Malaysia Digital Arrival Card is completed by entering information in an online form. Have these details ready before you start.
What the Form Asks For
MDAC asks for four broad kinds of information. Your passport and your travel bookings are the best references for filling them in.
Personal and Passport Details
- Your name, as it appears in your passport
- Passport number
- Date of birth
- Nationality or citizenship
- Place or country of birth
- Sex
- Passport expiry date
Contact Details
- Email address
- Mobile number with country or region code
Travel Details
- Arrival date and departure date
- Flight, vessel or other transport number
- Mode of travel: air, land or sea
- The place you are travelling from before Malaysia
Accommodation Details
- Type of accommodation
- Address where you will stay in Malaysia, including city and postcode
Tips
- Enter your details exactly as they appear in your passport and travel bookings.
- Double-check your passport number, dates and email address before you submit.
- Keep your passport beside you while you fill in the form.
- The form asks you to enter information; it does not ask you to upload documents.
